Student Travel Support

Please provide the information requested below and send it via email to schultzd@ornl.gov. Put the information in a single line in the email message, not as an attachment, with the message title "Student support." For your application to be considered, it must be received by March 21, 2008.

Please send it in the format indicated by the example: comma-separated entries. Do not leave out any entry.

Information needed (in this order):

Last Name, First Name, Institution, Advisor Name, Degree you are working on, Email address, Presenting Talk or Poster at DAMOP08?, Dates present at DAMOP meeting, Mailing Address

Example:

Doe, John, Generic State University, Jane Smith, Ph.D., jdoe@physics.genstateuniv.edu, yes, May 27-31, 1234 Englewood Dr., Pittsburg, PA, 99999

If, for some reason, you cannot send it by email, you may send it by hard copy to

Those selected for support will be contacted to transmit their U.S. Social Security or Tax Payer ID number to the DAMOP Secretary/Treasurer, David Schultz, in a safe way in order to receive a student support check at the DAMOP Meeting.

Please be aware that in the past, DAMOP policy has been to support only one student per advisor and priority has been given to students who were not supported last year and to those who will be presenting a talk or poster. Priority will also be given to students who provide the information in the format requested.
